Steps

This was four bare radios on one line. The most consequential choice on the site had the least design.
Each card is a <label> around a real <input type=radio>, styled through :checked and :focus-visible. Keyboard and screen reader work because nothing here is a div with a click handler.
Real inputs in a real form: a custom control that only submits via JS is a checkout that fails silently for someone.

Adding a note needs JavaScript — it pauses the video and captures the current frame. The notes themselves are always readable at /my/annotations.
